Cancellation and payment policy

We have set this cancellation and payment policy in fairness to the many patients waiting to be seen:

  1. New patient and testing appointments require 5 business days notice to cancel. Short notice or failure to attend an appointment will incur a charge of $300.
  2. Follow up appointments require 2 business days notice to cancel. Short notice or failure to attend an appointment will incur a charge of $150.

We require that you keep an active credit card number on file with our practice; we do not make exceptions to this requirement. We will charge all outstanding balances to this card prior to sending any statement. These balances may include, but are not limited to, deductible, co-payment, co-insurance, cancellation charges, no-show charges, charges for lack of referral, charges for inactive insurance, charges due to denials by your insurance company for non-covered services, and balances resulting from partial payments.
