How to become our patient

Please note that we accept only patients age 10 years and younger.

If you would like your child to become our patient, please start by completing this form. After that, these are the next steps:

1. We will send an email confirming we received your form.
2. We will review the information you provided. This can take a week or more. If we need more details, then we will contact you.
3. If everything looks good, then we will email you instructions and links for completing our new patient form and uploading required documents.
4. Once we have received your completed form and your required documents, then we will contact you to schedule a series of up to four visits.
