Before your visit, please remember to complete any questionnaires in CHADIS and invite a teacher to join CHADIS.
Copies of school and testing reports allow us to better understand your child. These may include an IEP (individual education plan), recent school testing or reports, and evaluations by other professionals. We request that you send these to us before the visit by using this secure link.
We strongly prefer that siblings not accompany you and your child to your visit as this can be a significant distraction. Children must be at least 9 years of age to remain in the waiting room without adult supervision (and our waiting room is currently closed due to Covid).
